Case presentation A 79-year-old woman presented with an abnormal posture of the trunk, which had a lateral deviation to the right side. She was admitted for severe depression three months ago and was given 15 mg/day of mirtazapine (MTZ). Three days after the administration of MTZ, she exhibited symptoms of parkinsonism like short-step gait, rigidity, and tremor.

Medical history She had high blood pressure since age 55, which was treated with 80 mg/day valsartan and 5 mg/day amlodipine.
