The clerk records the consultation and acts as a bridge between the physician and patient. However, as per a study, these reports as often error-prone and minimally regulated. And, at the end of the day, the physician is ultimately responsible for the medical record, not the clerk, so caution is urged in using their skills outside of the defined parameters.
