Air trapping is known as AUTOPEEP results from stacking of breaths, during inhalation air enters air sacs but due to lack of elastic recoil capacity when exhalation completes air sacs fail to exhale all air from air sacs so stacking of air occurs in air sacs. It causes impairment in oxygenation & ventilation. It causes haemodynamic compromise by decreasing preload & increase in peripheral vascular resistance. Increase in expiratory time, reduced respiratory rate 6-10 breaths/min & peak expiratory time >40 cm of water help in improving PEEP.
The complications of anaesthesia in severe COPD, mortality rate is 47%. It can be reduced by Proper Assessment, Proper Optimization, Proper Intervention and judicious perioperative management. Would you like to suggest any guideline for management for the same?
