I am posting very rare neuro ophthalmic manifestations of few cases called as tolosa hunt syndrome. I had little bit of interest in neuro ophthalmology few months back. The tolosa hunt syndrome patients who had complained about following Unilateral periorbital pain Hemicrania Diplopia of a sudden onset In examination the patient’s age is round about 50 years. He had exophthalmos, proptosis, complete ophthalmoplegia, nerve palsy, RAPD, impairment of sensory division of 1st and 2nd branch of 5th nerve, both eyes with plus 0.50 sphere, both eyes point were presbyopic fundi.
