A recent Medicare-based multicenter analysis evaluated the impact of the Bundled Payments for Care Improvement–Advanced (BPCI-A) program on gastrointestinal oncologic surgery . Over 90-day episodes, no significant cost savings were observed compared with non-participant hospitals ( difference-in-differences: $316; 95% CI –$2976, $2709 ). Similarly, there was no measurable improvement in readmission rates, mortality, or healthy days at home. These findings suggest that current bundled payment models may be poorly aligned with the complexity of oncologic surgery, underscoring the need for r evised frameworks that better capture both cost and quality dimensions in cancer care.
