Few months back a patient visited my clinic (previously he had medical consultations with almost all doctors and simply rejected with an option of referring him to higher center ) with heart failure, he was treated and stabilized, later he was on & off visiting my clinic for the same condition but during all these time patient did not stop tobacco chewing and high salt intake despite repeated counselling.
