In a randomized clinical trial involving 1247 infants across 25 community-based clinics, early daily full-body emollient application (before 9 weeks of age) significantly reduced the 2-year incidence of atopic dermatitis (36.1% vs 43.0%; RR = 0.84, 95% CI 0.73–0.97; P=.02) compared with controls. The protective effect was strongest in infants not at high risk (RR = 0.75) and further enhanced in households with dogs (RR = 0.68). Importantly, no increase in cutaneous adverse events was observed.
These findings suggest that routine emollient use in infancy may be a simple, low-cost intervention to lower the burden of pediatric eczema and its associated allergic comorbidities.
