A 12-year-old boy was referred for the evaluation of a scalp rash which occurred 2 years after an autologous graft surgery, for a severe burn. He suffered a third-degree burn to his scalp 3 years ago. The rash worsened progressively over the year with associated pruritus, erythema, and a thin film developing over the affected area. Previous topical treatments helped the itch but did not ameliorate his condition. The patient had a personal history of atopic dermatitis, and his mother had a history of hidradenitis suppurativa.

On physical examination; The lesion encompassed about 70% of the patient's scalp, radiating out from the vertex The rash showed diffuse erythema and weeping shallow erosions Overlying yellow-brown crusting and atrophic skin bordered the grafted area.
