Laparoscopic Roux-en-Y Fistulojejunostomy (RYFJ) is an effective surgical option to treat persistent fistula post-sleeve after the failure of endoscopic and radiological treatment. It is technically challenging and should be performed by expert bariatric surgeons at specialized centers. The most important criterion is careful patient selection. The surgery should only be considered in patients with adequate nutritional status and after the failure of well-conducted endoscopic management. Additionally, obesity can also lead to a higher incidence of ventral hernias.
