Hi patient aged about 60 years with history of uncontrolled diabetes  came with wound ?gangrenous changes in the foot . wound is covered with keratinised skin and whitish worms are coming out ,could you please make out differential diagnosis for this.

fbs 260 ppbs 380 on ex with glycogen X-R twice a day ,smoker.
