In diabetic foot, when there is wound in the form of a tropical ulcer in the sole especially big toe as with most of the patients and open wound in the dorsum of the foot due to injury or some times due to extension of infection from  the tropical ulcer, the measures to be taken include the debridement of the slough and dressing with proper ointments of choice of the doctor. Here povidone iodine with sucralfate ointment helps a lot. Mupirocin is good but costly. Always give Injection Tetanus Toxoid and if severe wound infection, Tetanus Human serum is to be given.

The antibiotics should be chosen by culture and adequate dose with duration should be maintained with parenteral preparation. Since anaerobic infection is common, metronidazole will be of use.
