A patient 46 yr old with h/o DM for 8yrs. was on metformin 2g /d and glimiperide 4mg /d .blood sugar was well controlled .but for the past one and half yrs he is having very high blood sugar both fbs and ppbs.he started on insulin and other oha .but still he is having very high sugar values with hba1c in range of 9 . For the past 3months he is on huminsulin 30/70 46iu morning and 26 iu evening.
Metformin 2.5g/d pioglit 15mg, sitagliptin 50mg. Glimiperide 2mg.and voglibose 0.2 tid.still is having fbs>250 and ppbs>300.what to do next.?he is not obese
