40 years male, MR and obese, hypertriglyceridemia, hyperlipidemia, had chest pain in night of 29 August, DM on saroglitazar, metformin, atorvastatin, ECG done and PPI and domperidone given. In morning came to me for the mild chest pain did ECG, found QRS wide, AVR mild ST elevation. Aspirin with clopidogrel given. MR keeps on working whole day and came back in evening with same mild pain again ECG done, found st depression and same AVR and some changes in V1. 2D Echo done NAD.
31Aug CAG done, RCA 90% block. Circumflex fully thrombosed, two stents introduced flow restored. How to diagnose this case? Earliest such is difficulty of patient.
