Drug reaction in HIV coe with TB

35 years Female detected to be seropositive was on ART Zidovudine + Lamivudine + Nevirapine. Later she started running fever detected to have Tuberculosis started on AKT and ART changed to Tenofovir, Emcitrabine and Efaviranz for convenience of dosage The CD4 count had also improved in three months of ART. 4 days later she developed severe rash all over body. Thinking AKT to be culprit AKT stopped.

Now that rash is gone AKT reintroduced this time pt is tolerating it well so reason could be because of Efaviranz. Need to restart ART can  the same be introduced .or a booster PPI added but that increases cost tremendously. What's the best regime. What will be the Drug reaction in HIV center of excellence(coe) with TB.
