A 40 yr old presenting with fever and fatigue one month after surgery for haemorrhoids was found to have a heart murmur on auscultation and referred to our super-speciality hospital. On examination, a diagnosis of Mitral regurgitation was made and blood cultures were sent. All blood cultures grew Enterococcus faecium resistant to Ampicillin, Gentamicin, Clindamycin and Rifampicin. It was sensitive to Vancomycin, Linezolid and Teicoplanin.

Diagnosed as infective endocarditis due to Enterococcus faecium Vancomycin was given for 2 weeks and fever subsided. Infective endocarditis has to be treated for 6 weeks. Question is whether any additional agent has to be added to Vancomycin since Vancomycin alone is not recommended?
