A 70-year-old hypertensive man fell down in the home, injured chest wall. X-ray was done which showed sub cut emphysema, rib fracture and air under the right diaphragm. The patient is eating well and is mobile, only complaining of pain over the right chest wall.

The query is how air entered under diaphragm? Already present or was it through subcutaneous emphysema?
