Immune checkpoint inhibitors (ICIs) such as pembrolizumab have revolutionized melanoma treatment but carry risk of immune-related adverse events (irAEs), including potentially life-threatening endocrine dysfunctions like adrenal insufficiency. A 62-year-old woman with Stage III BRAF-negative melanoma developed secondary adrenal insufficiency after three cycles of pembrolizumab, presenting with nonspecific gastrointestinal symptoms and systemic inflammation. Diagnosis was confirmed by severely low cortisol and undetectable ACTH levels. Discontinuation of therapy and hydrocortisone replacement stabilized her condition without melanoma recurrence. This case highlights the critical need for proactive monitoring and early endocrinology consultation to detect and manage irAEs effectively.
