A 2025 systematic review evaluating 663 women across nine randomized trials reveals that probiotic supplementation significantly reduces radiation-induced gastrointestinal toxicity in gynecologic cancer patients , particularly those with cervical and endometrial cancers . Benefits included fewer episodes of diarrhea, better stool consistency, and reduced need for anti-diarrheal drugs —with no major safety concerns reported. Prebiotics alone, however, showed little effect. While the findings highlight probiotics as a promising adjunct to improve treatment tolerance and quality of life , gaps remain regarding their role in chemotherapy, immunotherapy, and survival outcomes.
