Case Presentation An 84-year-old woman was brought to the hospital in August 2025 following a fall, head injury, and transient loss of consciousness. She slipped while attempting to hold her walking frame, striking the right side of her head. Paramedics found her conscious with a Glasgow Coma Score (GCS) of 15/15 and transported her to the emergency department. Presenting Complaints The patient reported a one-week history of abnormal involuntary movements affecting her right forearm and hand. These movements were insidious in onset, present during voluntary actions, and absent at rest.

Past Medical History Type 2 diabetes mellitus, diagnosed nine years earlier, treated with insulin with suboptimal glycaemic control. Atrial fibrillation, managed with apixaban.
