A 35 years old lady admitted after midnight with signs and symptoms of acute appendicitis according to ED  physician assessment confirmed by the ultrasound done on the emergency basis. Next  morning  I came to  assess  the  patient  to  find signs  of  acute appendicitis, LMP 3 weeks  back (ED did not ask about this), no pregnancy  test  was done, the  patient  convinced she has acute appendicitis  and seen  by  anaesthetist, I got  suspicion  something  is wrong  although  USG report  is  clear for AA, I reviewed the USG pictures to  find  the  structure  is  wider  tube  than  appendix  and small  mass inside  that tube, I requested  opinion  from  another  radiologist  who confirmed  my  suspect, pregnancy test  requested  before proceeding to  CT  abdomen.
