Let us again go back to that female T2 DM Patient. Drug naive patient with high f and pp bs. I put that patient on short-term one month Glargine + Metformin + Pioglitazone. After one month, the drugs were stopped except Metformin. Patient was on Metformin 2g only with good glycemic control. Now her lipid profile has relevant points: LDL- 130 HDL- 34 TG- 384 Euthyroid non-smoker, no alc intake occasional cold drinks that also now reduced. Non-lactating. Family h/o triglycerides not known.
Patient is adhering to LSM. I have put her on Metabolic statin Pitavastatin to be taken after dinner. Plan: - Recheck Lipids after 2 months. From my earlier experience, I know her LDL will come down to target. HDL will be somewhat raised (10 to 30%). If TG is above target, consider Saroglitazar, 4mg daily for 2 months and then review.
