Recently a healthy 14 yr old boy presented in very tense condition for last 5 days due to pressure to memorize book by heart. He was having extreme giddiness for 2 to 3 days and had the unsteady gait and complained to fall down on standing. Father gave the history of recurrent fever and generalized allergic rashes for last 3 to4 yrs which were under control with montelukast+cetrizine tab for last 3 yrs. Rashes recurred in 1 to 2 days on its discontinuation. F ather suspected giddiness and stressful condition due to this tab but hesitated to discontinue it suspecting recurrence of intensely itching rashes.
CBC, ESR ur r/m and c/s .na.k.cr.sgpt.ca were normal. How such case can be managed? Was the child having allergic rashes? Is it correct to continue montelukast+cetrizine for 3 yrs?What was wrong in ongoing treatment ?Allergic tests are needed or not in this case?
