A 37-year-old man on regular hemodialysis for end-stage renal disease presented with progressive dyspnoea and orthopnoea. The patient had nephrectomy to control hypertension 4 days before presentation. History The patient had been regularly undergoing hemodialysis three times a week for last 9 years after the rejection of a transplanted kidney. Examination He was severely dyspnoeic at rest. Pulse rate: 102 bpm, respiratory rate: 20 beats per minutes, blood pressure: 110/70 mmHg. Lung auscultation showed left basilar bronchial breath sounds.
Chest radiography indicated cardiomegaly with an air-fluid level at the level of the heart. Chest CT scan was obtained (3rd Image).
