A healthy 18-year old man was admitted to ED after a motor vehicle accident. He had sustained a head injury that resulted in intracranial bleeding at multiple sites, fractures of facial bones, and immediate traumatic mydriasis and ptosis in the left eye. Two weeks later he had profuse epistaxis from the left side of his nose. Examination Nasal endoscopy was normal. There was complete ophthalmoplegia, ptosis, and mild exophthalmos. The vision was restricted to light perception.
Left eye showed no chemosis, pulsation, or bruit. CT angiography was performed (refer attached images).
