A 56-year-old male was admitted to the hospital following a motor vehicle accident. During his stay at the hospital, his Hb dropped significantly without any obvious source of bleeding. The clinician inserted a nasogastric tube which returned at least 2 liters of black fluid. An upper gastrointestinal endoscopy was performed which revealed many large crater-like gastric ulcers (up to 30 mm). Biopsy of the ulcer showed broad pauciseptate, ribbon-like, slightly refractile fungal forms, which were highlighted with periodic acid-Schiff (PAS) stain. A Gomori methanamine silver (GMS) stain was negative.
