Case presentation A 79-year-old female presented with complaints of exertional dyspnea and palpitations. She also complained of night sweats, low appetite, and a weight loss of up to 4 kg in four weeks and was admitted to the casualty department for heart failure. She has no previous history of cardiac implications or COVID-19 infection.
Examination Vitals: Afebrile, heart rate – 98 per minute, BP - 120/7 mmHg ECG: Persistent atrial fibrillation (CHA2DS2-Vascular disease, age, sex category[A1] [AM2] -score of 3) Elevated natriuretic peptide levels No findings were observed in an extensive infective work-up, thoracic & abdominal CT, gastroscopy, and colonoscopy, ruling out any infection or concomitant malignant disease.
