Patient aged 49 year presented with swelling of 20×10 cm, side of the cheek. Swelling gradually has increased in size and was first noticed 9 year back, 5 year back fnac was done at AIIMS and was diagnosed as ca. (Can't comment on type as no reports are available ) and was declared inoperable and patient was said that she has 2 to 3 months to live but 2 years later on, she underwent another bx with IHC and was diagnosed as adenoid Cystic ca of minor salivary gland (cytokeratin and CD117 positive). CT scan showed the lesion was arising from ramus and condyle of mandibles and from imaging point of view put D/D for odontogenic cyst or adamantinoma.
Again as the pterygoid plates were involved, patient was declared inoperable. 4 year later patient now has presented to me with pressure necrosis of the skin for which debridement was done and tissue was sent for HPE. Now my pathologist has put…
