My patients is a 70 year old male, known case of trigeminal neuralgia on pharmacotherapy with varying doses of carbamezepine, oxycarbamezepine in combination with gabbapentine but has not undergone any intervention. Initially only maxillary and mandibular divisions were involved but off late he complains of pain in the lateral canthus of eye radiating to the occipital region. What could be the clinical implication of this new development?
Is there any new pharmacotherapy for the same? MRI brain doesnot show any discrete abnormality (tumor, MS) except looping vessel which is not compressing the nerve root. Is there any advantage of getting a 3D MRI done
