What is the recommended management for a Patient who took treatment for Latent TB (INH for 9 months) following mantoux and Quantiferon TB Gold positivity but sputum AFB negative 2 year back. The patient has history of occassional Upper Respiratory tract infection with frequent dry Cough. Now the patient came with Pharyngitis with ear ache with Productive cough with occasional streak of Blood. Lungs are clear on Auscultation and X ray appears normal.
CT taken before initiation of INH showed Apical Fibrosis. Is repeat sputum AFB required? What can be the further management?
