A 17-year-old female presents with fever (T39.0), vomiting and diarrhoea, widespread muscle aches and a confluent erythematous macular rash resembling sunburn all over her body. She is persistently hypotensive (BP 85/35 mmHg) despite 2L of crystalloid fluid resuscitation. She completed menstruating the day before and was otherwise previously well.
