Case presentation A 78-year-old man presented with a three-year history of an elevated leukocyte count with recent fatigue and anemia. He had received two red blood cell transfusions in the past two months. Medical history: Coronary artery disease and hypertension Clinical examination White blood cell (WBC) count: 75,000/uL Hemoglobin: 9.3 g/dL Platelet count: 71,000/uL WBC differential: 60 percent neutrophils, 19 percent lymphocytes, 15 percent monocytes, and 6 percent eosinophils His bone marrow aspirate shows mild erythroid dysplasia, 1% blasts with an increase in monocytes (14%) and eosinophils (7%) Fluorescence in situ hybridization (FISH) testing for the BCR-ABL translocation and quantitative RT-PCR for the BCR-ABL transcript were both negative.
