A 28-year-old girl having a cough with expectoration, fever, weakness and loss of appetite & weight since Feb 2017. She had consulted a physician. Sputum 3+ for AFB. Chest X-ray early cavities in Rt upper zone. She was initiated on AKT 4.2, follow-up chest X-ray showed well-formed cavities in Rt upper zone, Sputum was again 3+ for AFB. Meanwhile, she lost 5 kg weight (35-30 kg from Feb to June). Physician added Oflox to AKT 4. C & DST not done.
The patient is HIV negative. No any medical illness. No H/o contact with k/c/o MDR. She took medicine regularly as advised. CBNAAT was done at Dist. TB centre & it showed Rif resistance. Initiated on Cat IV regime as per RNTCP guidelines. Can this diagnostic delay be prevented?
