A 65-year-old male complains of two episodes of syncopal attack. He is a hypertensive controlled on medications. Known case of coronary artery disease, for which angioplasty has been done 3 years back. Triglyceride level is more than 600.

Carotid doppler shows 70 % stenosis near the carotid bulb extending into the right internal carotid artery with 40 % stenosis. Whether carotid stenting should be done in this case or not? If not then what should be the treatment?
