Surgery for a fungating and a bleeding sarcoma of the breast is a challenge. The term toilet mastectomy is often confused with sub-optimal mastectomy i.e. done for the emergency scenarios. However, like any other mastectomy, it requires removal of the breast along with nipple areola complex, along with pectoral fascia and axillary tail of Spence. The effort to have an R0 resection (microscopic freedom from cancer) is necessary in order to have an optimum clearance. This case demonstrates one such method of managing a fungating, bleeding sarcoma that needed an emergency mastectomy.
