In a robust multicenter, double-blind randomized trial involving 442 patients with primary acquired nasolacrimal duct obstruction (PANDO) , researchers assessed whether 0.04% Mitomycin C (MMC) applied at the ostium could improve results in non-endoscopic endonasal dacryocystorhinostomy (NEED) . The findings were striking— no significant difference in structural patency or long-term success between the MMC and control groups at both 1 and 5 years ( 94.6% vs. 95% at 1 year; 93% vs. 94% at 5 years ).

Common complications such as granuloma ( 4.5% ) and tube prolapse ( 4.0% ) were comparable. These results challenge the routine intraoperative use of MMC in NEED, emphasizing that meticulous surgical technique may be more crucial than chemical adjuvants.
