I have a 70 year old female patient with CRF - Urea 60 and Creatinine 4 with Anaemia Hb 6 gm%. She is on Oral Iron tablet, Inj. Erythropoietin 4000 IU given twice a week. On Anti-hypertensive clinidipine 10 mg. She has developed Proxysmal Nocturnal Dyspnoe since last two night. She had wheeze and BP use to shoot up. She was given bronchodilators and diuretic which helps. Other than at night she is alright during the day.
As she is anaemic, could it be the low Hb or could she be having CCF? I am looking at giving Inj. Iron and digoxin and furosemide. Can you kindly suggest your line of treatment and management? What do you suggest on the dosage of Iron, Digoxin and diuretics etc? Thank you for your valuable inputs.
