An 8-month-old boy presented with a history of moderate to high-grade fever for the past 10 days, which did not respond to prior treatment with cefixime and azithromycin. On examination, the child was markedly irritable and had bulbar conjunctivitis, strawberry tongue, and unilateral cervical lymphadenitis.

A faint maculopapular rash along with pedal edema was also noted. Investigations revealed raised total leukocyte count (TLC) and markedly elevated CRP, while blood and urine cultures were sterile.
