A 65 years old male patient presented with the complaint of an unhealing wound of left planter aspect. The weight of patient 57 kg. Loss of sensation & weight (about 8kg within 5month) O/E afebrile, pale, BP 140/90, pedal edema, pallor+++ (Hb8.6mg%), Pulse: 100/min, CVS: S1&S2+P2. Loud. CNS: Conscious, disorientation ++, drossiness RS: tachypnea ass with tachycardia. Wound: Foul smelling with serum discharge with pus.
Pus location on planter aspect of the left foot. H/O Uncontrolled DM (not responding to insulin) ECG: AWI BS: F=245, PM:540 Patient : Also undergone Angioplasty ( LAD:60% CAD:40%) blockages. Note: Patient admitted to Hospital (discharge after 6 days) Reason : Pus C/S Pseudomonas positive + staphylococci ++. What can be the diagnosis? What is primarily treatment? Whether is it curable or required amputation?
