A 65 years old man with old, MI( 8 years back), acute pancreatitis treated (1 year back) developed pain in upper mid abdomen not responded with tab.tramadol, patient had dissension due to flatulence but not passing in any way.Inj.ranitidine and metoclopramide were given, cold skin due to sweating in hot humid weather. BP monitoring as 125/85, 114/ 75in lying down position and 98/65 mmHg in sitting posture.pt.had taken 2 tab. Isosorbide Dinitrate and 2 tsf of antacids and antiflatulent mixture at home.
What is the next step of management in this case? Should I wait for response or immediately shift him to cardiac emergency? Please share.
