Pulmonary Artery Hypotension - A Common Unrecognised Problem Encountered During Spinal Anaesthesia, Poor Right Ventricle and Unrecognised Hypovolemia

Immediately after Spinal Anaesthesia, induced Hypovolemia and readjustment of fluids  begin— Along with Adrenal block and fall in catecholamine and Adrenal steroid levels. A common observation is mild Hypotension along with 90%- 92% spo2, even with  oxygen supplementation. Desaturaion even with oxygen supplementation— This gets corrected by fluid loading . But some times , Only a vasopressor shot Improves saturation and blood pressure .. This must be due to poor right ventricular contractility in weak anemic hearts  Or unrecognised excessive Hypovolemia.

Leading Inadequate Low pulmonary Artery Filling pressure = Pulmonary Artery Hypotension. Leading To LOW cardiac output and Systemic Hypotension.
