Case presentation A 42-year-old male presented with a one-week history of flu-like symptoms, including general malaise, fever, headache, muscle aches, and joint pain. He noted that his joint pain was moved from one joint to another over the past week. He also complained of multiple rashes on his lower limbs which started as papules and then evolved daily into large erythematous lesions with an area of paleness in the middle of these lesions. He had no medical history and recent travel history abroad. Examination There was a small area of cellulitis over the left calf.

In addition, there were multiple annular target-like erythematous rashes on the right lower leg that were 8 cm in diameter. There was a large, tender erythematous swelling on the left knee, but no other joints were affected. The rest of the clinical examination was unremarkable.
