An 18-year girl presented with 6 days h/o fever with chills and productive cough. No h/o burning micturition, pain abdomen, clay-colored stool, weight loss, and menorrhagia. Previous h/o two enteric fever, ATT. On examination, vitals are within normal range. Pallor is present. No lymphadenopathy, cyanosis, icterus and organomegaly. On investigating, Hb 9.1 g%. counts within normal range. MCV and MCHC decreased. The reticulocyte count is 3.5 %.
GBP shows mixed cells of microcytes, normocytes, and macrocytes. Tear-drop cells are present. Urine examination normal. MP smear and MPQBC Negative. Widal test negative. IgM typhoid test negative. Dengue serology negative. CXR findings are within normal limit. What are the various differential diagnoses? How to work up further?
