50 years, Male, had effusion, tried to drain, now after 10 years this is the picture of chest XRAY. Now he has developed Hypertension, DM, severe urinary bladder cystitis with spilited bladder wall.

Now c/o pain at Lt chest lateral side, and frequently passing urine day and night ( total 24 times) Blood sugar levels normal, BP normal. What is the cause of pain and frequent urination?
