The purpose of this review article is to enumerate surgery first approach (SFA) guidelines and whether the surgical first approach increases the post-operative orthodontic tooth movement. The primary aim of an orthodontist is to provide the good if not the best result from orthodontic treatment, whether it comes by tooth movement or by orthopedic or by orthognathic surgery. A surgery first-approach defines that orthognathic surgery precedes the orthodontic treatment, whereas the orthodontics-first approach indicates that the orthodontic treatment precedes the orthognathic surgery treatment.
