My patient is 70 years old with a known case of Diabetes mellitus2, Hypertension since 20yrs back . Present to my OPD with profuse sweating during night just half an hour after sleep and sometimes just after wake up . He is chronic cannabis abuser since 25 years. He is on metformin, glimepiride, pioglitazone, and also on insulin 15 u bd. He also takes Torsemide and Spironolactone 5/50.
Aspirin 75 od, Atorvastatin 10 od Metoprolol 25 od and Amlodipine 5 od Having a history of dyspnoea during upstairs. ECG, RFT, LFT within normal limits. What would be the diagnosis and management for this case?
