Topical calcineurin inhibitors are derivatives of ascomycin macrolactam and have immune-modulating and anti-inflammatory properties. They are used to treat atopic dermatitis by acting on the immune system of the skin to reduce inflammation, redness, and itching. Randomized trials support a beneficial effect of pimecrolimus for periorificial dermatitis (POD) in adults. However, efficacy data in children are limited.
It was observed that topical calcineurin inhibitors were associated with POD clearance in two-third of children, with mild and rare adverse events in a retrospective study of 72 POD children.1 The lack of a control group was an important limitation of this study because POD may spontaneously improve.
