American College of Obstetricians and Gynecologist (ACOG) practice bulletin redefined postpartum haemorrhage (PPH) and introduce tranexamic acid as an adjunct treatment to manage the PPH. Tranexamic acid binds to lysine receptors on plasminogen and plasmin thus inhibits plasmin-mediated fibrin degradation. Reduced mortality without increased thrombotic complications from obstetric bleeding was reported upon tranexamic acid treatment within 3 hours of delivery. Upon failure of initial therapy, ACOG recommends its use. For all cases of PPH, as per WHO recommendation standard PPH package should contain tranexamic acid.
