A 57 years old woman with diabetics, gout, osteoarthritis knee, hypothyroidism, dyslipidemia. All disorders controlled with medications c/o pain in shoulder back. On examination, that area is stiffened.

Massage of that area results in long belching followed by relieving the pain and stiffness. What will be the diagnosis and how to treat this case? Please advise.
