Warfarin is an oral anticoagulant commonly used in post valve replacement surgery, atrial fibrillation and other cardiac disorders to prevent stroke, deep vein thrombosis, cerebral venous thrombosis routinely in spite of there are various views regarding the usage in these conditions. In a case of ischemic stroke with thrombosis of a particular cerebral artery for prevention of further progression of thrombosis as well as recanalisation of already thrombosed artery there is no trial which is found in neurology literature.

So, as a novel method of treatment after five days of LMWH course, warfarin was started in a patient with thrombotic occlusion of bilateral vertebral arteries and basilar artery with  ischemic changes in the brain in MRI brain with MRA and MRV in 20, Feb 2015 and the dose was adjusted as per PT with INR results monthly.
