A r etrospective study (40 patients, mean follow-up ≥6 months ) evaluated anterior chamber (AC) wash with intracameral (IC) and intrastromal (IS) voriconazole in deep stromal fungal keratitis with endoexudates. Aspergillus flavus was the most common isolate ( 40% ). Despite treatment, only 17.5% achieved resolution with scarring, while 82.5% required therapeutic keratoplasty (TPK), and reinfection occurred in 30.3% of TPK cases. The findings indicate that AC wash with IC and IS voriconazole is largely ineffective as an adjuvant therapy for advanced fungal keratitis.
